We understand that after the death of a loved one families are often left with the onerous task of completing Canada Pension forms, insurance claims, health benefits, (reporting the death to employers, banks, trust companies etc.) at a time when these responsibilities can seem  insurmountable.  It is easy to justify putting these things aside until one feels strong enough to complete them. This often delays benefits to a surviving spouse or children and we know it helps to have this completed in a timely manner:

We will complete all forms required for:
Income Securities Program and the federal government related to Canada Pension and Old Age Security.

As requested we will:

  • Complete the application form for Canada Pension Lump sum death benefit.
  • Complete the spousal monthly benefit form.
  • Complete children's benefit forms.
  • Obtain and complete life insurance claimant forms.
  • Obtain and complete employer health, life and pension forms.
  • Give notification and supply certificates of death to any bank, trust company, union.
  • Deliver all forms once completed to their required locations.
